It does appear the lost MMR was due to a rollback as a result of players from a match you were in being penalized for cheating, etc.
Rollbacks can happen some time after the match has finished, however you should be notified in game of this.

I'd perhaps suggest cleaning the disc with a cloth or suitable wipe to ensure it's clean before trying again.
Are you also able to try the disc on another PS4 system? If the issue occurs on another system, this would further suggest the issue is with the disc.
