Getting a bit confused why a manual check every so often on the BH worlds isn't being done, Jagex has shown they can have real time heatmaps during DMM, it wouldnt be hard to have a few people on staff check on them every so often for anomalous gatherings away from the standard Edgeville BH pking.
We do clear them out often but there's the whole rest of the game to take care of too, and I'm sure you'd really start to notice rather major issues if we applied your method to our way of working. It's worth noting that BH Emblems would be much worse than what you're seeing if we didn't do anything.
As someone who has been pking since 08, it's pretty easy to tell when someone is AHK/scripting or actually good at the game. It's literally impossible to be able to click that precisely/switch prayers that clean in the following clips:

I really dont get it, they always have the same exact fucking look, with the same name, how does this shit not get fixed?
They're not exactly the same, very subtle differences can cause one solution to not work, and it's a very fine line of capturing and muting legitimate players.
Hey, there was a handful of accounts locked on Friday morning for precautionary checks to ensure that everything was in order. Whilst the majority of accounts were unlocked within minutes, 9 accounts remained locked over the weekend. I've now added missed membership to these accounts and unlocked them - please do note that an account lock doesn't mean you will have "lost their accounts and all their work" - they can simply be unlocked via the website.
Ive sent reports in and a tip off email about a certain player that was openly botting spiritual mages/warriors in their psd on 3 accounts and he wasnt banned.
Its very discouraging to report players that abuse the game (and then make real world profit off lf it) then get told to just make more reports.
I understand your frustration, and whilst we do miss some accounts or refuse to take action, this is due to lack of hard evidence. Reporting accounts does keep bringing them to our attention, so when there is enough evidence to support the claim, we can take decisive action.
Honestly it can't even detect autoclicking to train skills on RS. So many players with over 2b xp that I've seen do it and are quite outspoken about it.
We apply an average of 71 offences per day to players who auto-click in RuneScape (not counting OS). Whilst it's something we'd like to more proactively tackle, there's more damaging bots which we need to maintain focus on, but rest assured we do take action. Please report any players you suspect of auto-clicking too, as they will be reviewed on a daily basis.
We'll only ever take action against an account for interactions with our game, and not for simply running a program on your computer.
Will take a look at these areas, but please do continue to report each account as we review all and tweak Bot Watch based on reports too.
They're cleared on a daily basis. Unfortunately we can't preemptively ban an account for botting.

Does jagex try to go further and find out which accounts are benefiting from the bots?
Yep - these are usually picked up in our more in-depth Real World Trading checks, especially for gold farming bots such as these.
