u/CatBlips_Epic These buttons do not show in the Combat Mode HUD Layout tool, only the Creative one. In the past they showed in all.

The “Creative - Toggle Phase” and “Creative - Heatmap” are not in the Combat HUD mode, you can do them with controller input.
