The easiest way to find it is through the game. From the Main Menu go into the Content Manager, then find the Map Theme category on the list and find the theme in question on that list. You can click the Workshop icon to open the workshop page for it.

To easily find the local files for the game, right click your game in the Steam library, select Properties, go to the Local Files tab and select Browse local files.

Since you have Ultimate mod you also don't need No Fires or Pollution Remover mod. You should always avoid having mods that overlap like that as mods that do the same thing tend to conflict.
Try setting up a custom size pagefile of 32 GB. See this guide:

Sounds like you had a mod that enabled achievements with mods, which is either gone, disabled or broken.
I use the mod Steamy, but afaik Achieve It and Mod Achievement Enabler both work as well.
That sounds like an issue with the launcher. Try reinstalling it following the steps in this thread:

Did you manually move the files? Or did you have Steam move the game?
Sounds like a broken mod. Start by checking all your subscribed mods against this list of known broken mods. Remove/replace any you might have.

Circular means it has a round footprint. It can be handy sometimes.
If I remember correctly Fire Hazard is an on/off setting. If it's 0 then it won't catch fire, if it's 1 it might catch fire. Been a while though, so I might be off. :P I always leave them at the default.
If you remove the launcher following the steps in this thread, then it tries to install automatically when you press Play in Steam. Might be worth trying again, I saw someone yesterday where it worked the 2nd time. No idea why the first one didn't do the trick, but while you wait for the email, that's worth a try again I guess.

Most likely a file for the launcher has been corrupted, either just randomly (it happens, Windows usually just fixes normal files) or while trying to update the launcher. Not sure what's causing the reinstall to not work. :/
Try adding these two mods, enable them, restart the game and check that the Safe Mode options are still enabled.

Let's start by removing old broken mods. Remove these and get the replacements where available:
- Automatic Bulldoze v2 - get version 3 instead
- Extended Public Transport UI - this was added to the game long ago, the mod is not needed and likely broken
Then get the Loading Screen Mod, make sure to enable it and then go into Options. Find it on the left, then find the Safe Mode options and enable them all. Then try loading your save.

Unfortunately I'm not able to help with the launcher beyond the steps to reinstall it. So if that doesn't work, then you will need to contact Paradox Support. Hopefully the confirmation email appears today.

Where did you upload it to? Maybe Steam doesn't like the site. You can try and replace the . in the url with a space instead. Then Steam should ignore it.
