These types of posts are constantly removed. There are only so many moderators, though. Also, very few people actually report such posts in the first place, which makes it harder to deal with them.
It is unlikely that your name change is the actual problem - our account database has always stored your PSN as a numerical id, not the display name. There are number of other things that could be going on, though. You are welcome to send me a bungie.net private message (I'm "Achronos" there too, user id #1) - let me know the clan, who you expect to be the founder, your current and former PSN name. That way I can look into it.

The new beta fireteams feature does not require a friend request to be sent. You see a fireteam requiring a player, you join it, and the fireteam owner can issue you a platform invite. You'll get an invite on your PS4/Xbox One directly to that fireteam, just like if they were on your friends list and invited you to their fireteam. Unfortunately, that doesn't work for PC, we're hopeful we can get that added eventually.

Yes, the Destiny Companion allows PS4 and Xbox One users to directly join the in-game fireteams without being on someone's friend list or in the same clan. It is somewhat buried in the UI right now, we're working on it.
A way to not only make a fireteam, but to message them and mass invite them would be all I would need to exclusively use this app for group finding.
Currently it’s a fairly unintuitive system. I shouldn’t feel like I need to search youtube for a guide on how to use the Find Fireteam portion of the app
Messaging will be added before it comes out of beta. You can already mass invite them to your game fireteam once they have joined. There are a lot of UX bugs still needing to be fixed as well to make it more intuitive to use.
The entire reason we shipped it in such an early state was to get feedback from users about it. It makes it much easier to iterate when we're building on a solid foundation instead of ripping out things we built but weren't really the best for users. We're going to be trying that kind of thing more often, as much as possible.

Logged in each of my 3 characters and loaded DIM on Chrome - still get an error.
It takes a few minutes sometimes. Also, if you have characters on more than one platform, you likely need to log into them to (some 3rd party apps load all your accounts instead of just the "current" one like the official companion does).
At any rate, we are trying to get this fixed as soon as possible.
At least for destiny companion app I don’t think this is true. I still haven’t logged into any of my PS4 characters, but after logging into my all of my pc characters the issue was fixed. I don’t use Ishtar commander so you may be right with that one.
The official app only loads one account at a time, you have to switch between platforms. Many 3rd party apps load all linked accounts at once, which is why they require a login to all characters for the workaround to work.

I was against a Sentinel that spammed bubble and suppression grenades. It was like a budget tether.
I saw a few those when I was playing over the weekend. They were sure surprised when they got booped out of their bubble by my Tractor Cannon.
I don't always run a shotgun in PvP, but when I do, it is a Tractor Cannon.
When were you able to access the app again? I've been trying on and 9ff all day but it keeps saying "Disabled for Maintenance"

So, if that imgur picture is the current state of your account, it appears that you have all 3 account types linked to your bungie.net profile. Assuming that the blizzard account linked to it is the wrong one, what appears to have happened that you managed to unlink your "real" blizzard account. This likely happened when signing back in with blizzard, you probably signed into the wrong blizzard account and you were asked if it was your first bungie.net profile, you said no, and signed in with an Xbox or Psn account. That caused the wrong blizzard account to be linked to your profile.
You can't unlink it because you can't unlink something you are signing in with. But you should be able to repair it by signing into bungie.net with the same account you sign into the game with (be sure it is the right one). One of two things will happen. As far as I can tell, I've tried all of the steps you listed already, but to be sure I just ran through it all again. I successfully unlinked the "placeholder" account, and then signed out. I should mention that unlinking this account even required me to "sign in" using the credentials for the correct battlenet. I signed back in using my Battlenet login, and it prompted me as you described - I chose "no, I have another profile" and logged in with my Xbox GT. It gave another prompt to link the two profiles, and it did indeed show my correct "Refl3xes" bungie profile with the right emblem and GT associated with it. After I linked them, it took me back to the home page of Bungie. Sure enough, though, it all ended up exactly the same... there is an account shown linked, presumably the one I Just linked, but it appears without a name and looks like a placeholder. It even has "show on public profile" checked, but when you view my profile here https://www.bungie.net/en/Profile/254/965833/Refl3xes you only see my Xbox GT linked.
That's a link to the current status of my "accounts and linking" page after running through the steps you listed, as best as I could.
If I log out and log in using my Xbox GT, it takes me to that profile. If I do it with my Blizzard credentials, it takes me to that profile as well - but it still shows the dummy profile linked. And I still can't access the emblem, just checked :(

I'll look into it more when I get in the office Monday, but I'm curious - what blizzard account are you intending to link to (name#number)? You can direct message me if you don't want to say publicly. If you can still sign into the game on PC and everything is correct, then you aren't signing into bungie.net with that same account. Do you actually sign into the game on PC, or is the Blizzard launcher already signed in when you boot it up? I suspect you have two accounts with very similar logins and identical passwords, or something like that. Because if you can sign in via Xbox, and successfully unlink the unnamed Blizzard account from your settings page, hitting the "link" button from there and using the correct blizzard account should do the trick.
Anyway, if you give me the "correct" blizzard account name, I can check to see where it has been, and I can confirm a few details. I can't fix the the linking state itself (security policies), but I can at least verify some deta...

Don't worry about it - accounts are sometimes super tricky, especially in a multi account world. I'm glad everything is working now!
Glad you like it - I personally spent a lot of time on the "invite directly to game" function of it, I'm really proud of it. But it only works so well because of how awesome this community generally is.
We tried out something new this time that only turned off a few but not all of the Destiny 2 systems. TransferItem was not something that needed to be shut off for this type of patch (i think vendors, socketing, and talent grids were shut off, IIRC). We can't always do it, but nobody likes having to shut off the API for hours on patch day. Hopefully our changes will make that the exception rather than the rule.
IT Buzzword bingo aside, the problem with it isn't a load issue (though high concurrency does make it worse, it isn't the cause), there is a low level bug in the part of the service that it uses to communicate with the game services. We do know what is causing it, the fix is somewhat delicate though, and can't just be slapped in. We are working on it though and hope to fix it soon.
There is a bug with the part bungie.net that communicates with the game that manifests more frequently the higher concurrency gets. We are working on the problem.
