My audio output was on something "sonic" when I checked and changed it to what was suggested and the game sounds fine now.

Hi, this was fixed for me by following the instructions from one of the comments in this thread.
I can't tell you now whether it's the game or the Xbox settings as I have no idea what my Xbox audio settings were before
If it helps I had filled reset the Xbox I'm using UR Beats plugged directly into my standard Xbox one controller

Hi everyone! We've had our eyes on this thread (and others like it) and were hoping you could give us some additional information so we can take a deeper look:
- Does this still happen after you do a full reboot of the console? (hard press on the front button and disconnecting power cables?)
- What is your exact hardware setup? Are you using headphones through HDMI? Optical? Any additional information is appreciated.
- What are the audio settings on your Xbox? (Settings, General, Volume and Audio Output)

Objective 2 wasn't tracking properly, so it's now working as intended; kill 50 Juggernauts with Otter as your Coalition Operator.
For the last mission tracking, you can complete the same operation 4 times to complete it.
If you're still having any problems completing these, let us know so we can take a look.

You can click continue with no issue - you won't see your stats or rank reset.
