I feel misled, i got 5 barrows totems instead of "a barrows item" which feels like it implies an armour or weapon, which would have been really nice and time-saving for the morytania diary grind. oh well.

If they hotfixed it to say barrows item x1, why couldn’t they hotfix it to say barrows totem x5?

Because there is a chance to get a random Barrows item, the fallback for failing the roll is the 5 totems.
