What is the most op (small)change riot could implement to a champion?

Nothing like "karthus ult now deals true damage" that is going a bit too far.

More along the lines of:

"Azir's soldiers now apply nashors passive"

"Fiddle ult cast time decreases with rank, down to 1 second"

"nocturne passive/reksai q aoe now trigger smite passive burn on monsters"

"malzahar voidlings now take reduced damage from AoE effects, similar to yorick ghouls"

Ezreal Q can now crit

We tried a version of this when we were doing the item update. If his Q "Crit" then it fired a 2nd, identical projectile at almost the exact same time. Unfortunately it didn't really work and wasn't very fun.

Did you ever try something like mage spell crits or spell crits or something?

Yeah we tried it with an item at some point. I wasn't there for that dev though so I don't know what the conclusions of it were.
